NOTE: The process outlined below is applicable for eligible Zambians who reside in the United States or Countries of extra accreditation. The countries of extra accreditation are: Belize, Costa Rica, Dominican Republic, El Salvador, Guatemala, Haiti, Honduras, Mexico, Nicaragua, and Panama.

Passport Application Procedures
  1. Submit your request for passport application forms by e-mail to consular@zambiaembassy.org or by mail to:

    In the subject line of the email message, enter the following information: REQUEST FOR PASSPORT FORMS or REQUEST FOR TRAVEL DOCUMENT APPLICATION FORMS (whichever is applicable)

    Include the following details:

    1. Applicant’s name in full
    2. Date of Birth
    3. National Registration card number (in the case of a minor (aged under 16 years old), provide one of the parents’ National Registration Card number)
    4. Mailing Address
  2. Upon receipt of your request for passport application/renewal forms, the Embassy will mail you the Passport Application package. Included in the package will be an Application Instruction page.
  3. Passport Application fees listed below should be submitted along with the Application package when the applicant comes to the Embassy in Washington, DC for the scheduled in-person Consular Interview:
    1. 32-Page Passport – USD $80.00
    2. 48-Page Passport – USD $100.00
    3. Travel Document of Identity – USD $20.00

    Other Fees:

    1. Penalty Fee for a Lost Passport - USD $20
    2. Penalty Fee for a Lost Travel Document of Identity – USD $20
  4. For more information, please e-mail your requests to consular@zambiaembassy.org. In the subject box, enter “Zambian Passport” and address your e-mail to the “Attention of the Consular Section”.
Pending Applications
  1. Send an e-mail requesting a status update to consular@zambiaembassy.org. In the subject box, write "Passport status update". In the body of the e-mail, include the following:

    1. Applicant's name in full
    2. Date of submission and receipt number
    3. Applicant's telephone number
  2. Followup should be by e-mail only. E-mail to consular@zambiaembassy.org
